Forms for Credential Files


Credential Files are essential for Education majors to secure employment upon graduation. Your Credential File is housed in the Office of Career Services.

Student Personal Datasheet:

This form is completed by each student and becomes the first page of your credential file. This page is not optional. Your credential file is not complete without this form.

Alumni Personal Datasheet:

Alumni that are in need of sending their credentials are encouraged to update their file with current form and reference letters. The Datasheet is one of the forms you will want to update. It will give your potential employer your current address and contact information and allow you to showcase your work experience.


Coursework Summary:

This is an optional form. You may choose to include a copy of your transcript instead of completing this form. Transcripts are only available from the Registrar's Office, 651-638-6250.
