Proof of Insurance

If you're covered by a family or individual heath insurance plan, you must complete an online waiver to opt out of the plan Bethel is making available.

Students can access the waiver through Blink (Banner icon > Student tab > Create Student Insurance Waiver link).

The waiver must be completed by Sept. 14, 2009, or students will be automatically enrolled and billed $750 through their Bethel account.

Undergraduate students enrolled in the College of Arts & Sciences are required to demonstrate they have health insurance with a minimum of $50,000 coverage for each accident or sickness.

This insurance can be through a family plan, an individual plan, or the accident and sickness plan Bethel provides through Student Assurance Services (SAS).

Who is required to have health insurance?

All Bethel students are required to have health insurance.


Why does Bethel require health insurance?

At Bethel University, we try to do all we can to help students successfully navigate through college life. Sometimes that means managing unexpected challenges due to illness or injury. Having health insurance helps students get the care they need and can keep medical expenses from derailing a student’s educational goals.


I have health insurance through my family plan or an individual plan. Do I need to do anything?

Yes. All students at the College of Arts & Sciences who have insurance with a $50,000 or higher maximum benefit per accident or sickness must complete the online waiver form, even students enrolled only for one credit.

When is the deadline for completing the waiver?

Students are required to submit proof of insurance by completing the online waiver form before the 10th day of fall classes (September 14, 2009). The waiver must be completed annually thereafter.

What if I do not complete the waiver by the deadline?

If you miss the deadline, you will automatically be enrolled in the plan Bethel is offering. You will be billed $750 for the annual premium. It will be difficult to reverse this charge with the insurance company.


Who is eligible for the plan Bethel is offering?

All Bethel University students who are enrolled for credit.


What is the cost of the plan Bethel is offering? How do I enroll and pay for it?

The plan is available for $750 for one year of coverage (August 10, 2009 – August 9, 2010). You can pay for the plan through your student account. Student loans can also be applied to this cost.

If you would like to enroll in the plan, go to the online waiver form and select the first option.

What insurance provider has Bethel selected? What are the details of the plan?

Student Assurance Services (SAS) is a local plan administrator that offers affordable student health plans and a high level of customer service. We are pleased to partner with them to offer these plans to Bethel students.

What are the insurance requirements for student athletes?

Student athletes are required to have health insurance that a) meets the NCAA required minimum of $75,000 for each accident or injury and b) does not exclude intercollegiate sports accidents or injuries.

This insurance can be through a family plan, an individual plan, or the plan Bethel is offering. Student athletes are required to submit proof of insurance by completing the online waiver form by August 1. To access the waiver, see the instructions in the box above.

If they do not complete the waiver, they will be automatically enrolled in Bethel's accident and sickness plan as well as the appropriate sports plan, and will be billed through their Bethel student account for both plans. Rates for the the plans are:

· Accident and Sickness Insurance: $750

· Football, Hockey, and Soccer: $500 additionally

· All Other Intercollegiate Sports: $300 additionally

(Please note that the NCAA will raise this deductible to $90,000 for the 2010-2011 school year.)
